Important Immunization Reminder
If you have been notified by the school that your child needs updated immunizations, those records must be submitted to the school office by 8:00 a.m. on Tuesday, January 6. If this information is not provided, your child will not be allowed to attend school until the required documentation has been received.
If you choose not to have your child immunized, you must complete the KCI Immunization Form that is attached. Please note that completing the KCI/Immunization Form is required by the State of Kansas and must be completed annually.
When completing the exemption form, please look under Documentation – Other Immunization and select one of the following options:

• #1: Written statement from a doctor (documentation must be provided), or
• #2: Religious exemption
